The state of Kentucky sends this notice to us every month along with the form for paying the state payroll taxes.  I wonder how much money Kentucky taxpayers have spent for an efficiency expert to determine that it makes more sense to put this notice in with every payroll tax form than just to have its clerks separate the stubs from the checks.  Just thought you might like to see your tax dollars at work!
